Ministry of Health will invest 23.6 million reais in oncology

Improve the quality of laboratory tests and encourage the expansion of the offer of preventive cervical cancer testing in the range of 25 to 64 years. These are the main focuses of the measure adopted by the Ministry of Health, published inthe ordinanceestablishing the National Qualification in Cytopathology (QualiCito). The incentive for the costing of QualiCito predicts the increase of the value of the cytopathological examination by 5%. In addition, these tests, when performed in the age group of 25 to 64 years – for cervical cancer – will have an additional value of 10%.

Another financial incentive provided is the quality control of the exams. To this end, the Ministry of Health will invest, in 2014, about R 72.6 million for cytopathological examinations. Since 2010, the Ministry of Health has invested R$ 223.5 million in this action.

In 2012, 10.9 million cytopathological tests were performed within the SUS, of which 8.5 million were in the priority range (25 to 64 years). Of the total number of tests performed, 77% were performed in the priority age group, which represents 68.3% of the target audience, when considering all women between 25 and 64 years of age who take an exam every three years. This is a screening condition after two normal tests.

The creation of QualiCito is a measure that integrates the list of actions that the Ministry of Health has been carrying out since March 2011, when the Federal Government launched a set of measures to strengthen the National Programfor The Control of Cervical andBreast Cancer. In the cervical cancer control axis, the objectives of the plan are: to ensure access to quality preventive examination for all women aged 25 to 64 years and to qualify the diagnosis and treatment of precursor lesions of cervical cancer. Therefore, among the lines of action outlined to achieve these objectives is the quality of cytopathology tests. In 2012, the Health Care Network for People with Chronic Diseases was launched.

The quality of the cytopathological examination depends on the action articulated between states, municipalities and the Union. The collection is done in the health unit, through the transport of the slides to the laboratories where the analysis of the slides is performed, until the delivery of the results of the tests to the user – in a timely and appropriate time – for the screening, early detection and treatment of the disease.

In addition to improving the quality of cytopathological tests offered to the population, the Ministry of Health wants to stimulate the improvement of quality standards of public and private laboratories that provide services to the Unified Health System (SUS) and promote permanent education actions of health professionals, as well as monitor quality indicators.

In Brazil, cytopathological examination is used for cervical cancer screening (Pap smear), especially in the 25 to 64 years age group, which concentrates the highest occurrence of high-grade lesions that can be effectively treated so as not to evolve into cancer.

For 2012, the National Cancer Institute José Alencar Gomes da Silva estimated the incidence of 17,540 new cases of cervical cancer in Brazil, with an estimated risk of 17 cases per 100,000 women. Withoutconsidering skin tumors (non-melanoma), cervical cancer is the most common in the Midwest (28/100,000) and North (24/100,000) regions, followed by the Northeast (18/100,000), Southeast (15/100,000) and, finally, the South (14/100,000).

SUPPORT– The public or private laboratories that provide services to the Unified Health System (SUS) participate in QualiCito, in types I and II. Type II units receive 100% of positive samples and 100% of unsatisfactory results, in addition to 10% of negative analyses.

Part of the exams will be reevaluated by an external laboratory, qualified to meet the internationally validated quality criteria, with the ability to release the exam in less than 30 days. These establishments will receive the appeal in a single installment.
